Electronic Signature Declaration

By using an electronic signature via Novotech’s nominated electronic signature system, I certify that,

1. I understand the account and/or session being used to apply my signature is unique to myself, legally binding, and equivalent to my handwritten signature;
2. I have satisfied the requirement to verify my identity with the Company; and
3. I agree to protect the confidentiality of my account and/or session being used to apply my signature, from third-party use or modification and immediately notify upon becoming aware of fraudulent use, modification, or unauthorized access.
